Our Simple Referral Process

Simply fax the patient information to us at 262-780-0717 or send it via e-mail to mail@healthreachrehab.com. Include basic information such as patient name, contact information, diagnosis and the reason for therapy. We'll take it from there! We will confirm insurance information and will contact the patient to schedule the initial visit. We will provide you with pertinent information through the course of therapy.

Once you refer an adult, adolescent or pediatric patient to HealthReach Rehab for physical, occupational or speech therapy, you can be confident:

• Your patient will get the best, most professional care.
• You will receive regular communications and status updates about your patient.
• You will receive a discharge summary.
