Community and Home Based Therapy Services
We still make house calls!Since 1994, HealthReach Rehabilitation Services has been making House Calls. We provide traditional and specialized Physical, Occupational and Speech Therapy to disabled adults and seniors who live in the community. Unlike Home Care Agencies, HealthReach clients do not have to be homebound to receive our services. HealthReach accepts Medicare, Medicaid and most private insurances

Wellness Programs offered to the Community
Falls Prevention:
- The Safe Living Program is a comprehensive assessment of a person's physical and mental abilities while performing daily tasks in their home. Establishment of home exercise programs as well as recommended home modifications increase the person's safety and independence.
- The Community Based Falls Prevention Program includes a multitude of preventative, rehabilitative and maintenance strategies that complement Community Facility programs.
Exercise and Fitness Classes:
- Individually tailored traditional and specialty exercise classes such as The ROM Dance, Tai Chi, and Cardiovascular Fitness.
- Fitness Equipment Assessments for Facility Room Design and Individualized Fitness Programs for community residents.
Employee Injury Prevention:
- Group presentations for caregivers on topics such as Back Care, Floor Transfers, Bus Transfers, Identifying Falls Risk Factors....
- OSHA compliancy including Job task Assessment, Pre-Placement Testing and Functional Capacity Evaluations.
