Safe Living™ Program

What is the Safe Living Program?

Designed to provide a comprehensive assessment of an individual's physical and cognitive functioning as well as assess their home environment for falls risk factors. Through the Safe Living Program, HealthReach is responding to the growing need to help keep individuals safe and as independent as possible so as to reduce the likelihood of needing more assistance or relocating to a more supervised setting.

What types of individuals can benefit?

The Safe Living Program can be beneficial for anyone of any age or dysfunction. The following are specific symptoms that are indicators of potential problems that may affect a person's ability to be safe or independent.

  • Weakness: sudden or insidious onset
  • Complaints of fatigue or signs of shortness of breath
  • Increased complaints of pain
  • Difficulty while walking related to weakness or balance problems
  • Dizziness or complaints of motion sickness
  • Increased forgetfulness
  • Needing assistance with dressing, bathing or other self care activities
  • Needing assistance with meal preparation or home management skills
  • Difficulty lifting, holding or moving objects around the home

What does our program include?

A specially trained therapist does a three-part assessment.

  • A comprehensive assessment using standardized tests that assess a person's physical and cognitive functioning.
  • A complete assessment of the person's home with special emphasis on the main living areas, entrances and stairs.
  • Recommendations for home modifications, equipment purchases, community referrals and establishment of individualized home exercise programs.

How much does this service cost?

This type of assessment is typically covered under Medicare, Medicaid or private insurances. A physician's referral is required for the assessment to be covered by insurance.
